Program partners are agencies that:
Provide health programs and services
Work with an adult or older adult population
Would like to offer their clients or employees an evidence-based program shown to help participants improve their lives

Become a Delivery Program Partner:
A delivery partner is an organization or agency that coordinates the various aspects of our evidence-based program delivery. The delivery partner is responsible for training facilitators, which consist of the organization’s staff members or volunteers. Our delivery partners enter into a contractual agreement with our agency to help expand our program infrastructure. They are responsible for planning and monitoring the implementation of the programs.
Currently, we offer the opportunity to become a Delivery Partner for our Take Charge of Your Health Programs. Take Charge of Your Health is the Self-Management Resource Center’s Chronic Disease Self-Management Program (CDSMP). Take Charge of Your Diabetes is the Self-Management Resource Center’s Diabetes Self-Management Program (DSMP). Each program is licensed through the Self-Management Resource Center SMRC.
