Program partners are agencies that:

  • Provide health programs and services

  • Work with an adult or older adult population

  • Would like to offer their clients or employees an evidence-based program shown to help participants improve their lives

Become a Delivery Program Partner:

A delivery partner is an organization or agency that coordinates the various aspects of our evidence-based program delivery. The delivery partner is responsible for training facilitators, which consist of the organization’s staff members or volunteers. Our delivery partners enter into a contractual agreement with our agency to help expand our program infrastructure. They are responsible for planning and monitoring the implementation of the programs.

Currently, we offer the opportunity to become a Delivery Partner for our Take Charge of Your Health Programs. Take Charge of Your Health is the Self-Management Resource Center’s Chronic Disease Self-Management Program (CDSMP). Take Charge of Your Diabetes is the Self-Management Resource Center’s Diabetes Self-Management Program (DSMP). Each program is licensed through the Self-Management Resource Center SMRC. For those who find the licensing policies and fees to be a barrier, you can contact us to discuss how to become a delivery partner under our organization’s license.

IL Pathways was established to help connect nationally recognized evidence based interventions/programs to participants throughout Illinois. To add your program there are some key things that we would ask to put on our site:

  • Program logo, name and description

  • Length of the program, including days/hours

  • Major program objectives; how the program helps participants

  • Frequently asked questions

  • Contact information

  • Information about recruiting facilitators for the program (if interested)

  • Information about recruiting potential host sites for the program (if interested)
