Take Charge of Your Health and Take Charge of Your Diabetes are evidence-based programs originally developed at Stanford University. Participant results have shown improvement in health outcomes, better quality of life and reduced disease complications. Due to improved self management of diabetes and other chronic conditions, these classes can also help to reduce health care costs.
Workshops are conducted in trusted and accessible community settings. Classes are led by two leaders trained in the Stanford University model. Fidelity to the class structure and design is a requirement.
Take Charge of Your Health/Tomando Control de su Salud – Information and self-management skills for older adults with a chronic disease, and their caregivers.
Take Charge of Your Diabetes/Tomando Control de su Diabetes - A free program offered to all interested participants. Participants may currently have diabetes or be pre-diabetic.
Take Charge of Your Diabetes Plus - A registered dietitian leads the classes, provides two individual assessments to collect clinical data related to the participant's diabetes and helps participants identify educational goals to improve outcomes. Outcome data is shared with the referring physician. Requires that participants have a referral from a doctor. Scroll down for further information regarding our referral system.
