About the Class​

Take Charge of Your Health: Chronic Conditions empowers individuals facing ongoing health issues like arthritis, chronic pain, diabetes, and mental health challenges. These conditions can impact daily life, but you don’t have to navigate them alone!

Engage in an experience designed to equip you with the confidence, motivation, and practical skills needed to effectively manage your health. Discover strategies that will transform the way you approach your condition and enhance your quality of life.

Workshops are also available in Spanish.

Duration & Format​

  • Six weekly sessions, 2.5 hours each (with a built-in break)

  • Participate in person at community and clinical locations or virtually with technical support available

Topics 

  • Nutrition and healthy eating habits

  • Physical activity and exercise

  • Pain and stress management

  • Effective communication with family, friends and health care providers

  • Problem solving, goal setting and many more

Who Should Join​​

  • Adults with ongoing health conditions and their caregivers

  • Adults whose health problems have begun to interfere with daily activities

  • Adults who have had challenges following health recommendations

  • Primary focus: Participants aged 60 and over, as well as adults with disabilities, but individuals of all ages are welcome to join. Caregivers of those with ongoing health conditions will also find the program beneficial.
