Partner with Illinois Pathways to Health (ILPTH) to bring evidence-based health promotion programs to communities across Illinois. As a Delivery Partner, your organization delivers proven health education that helps individuals manage their health, reduce risk, and improve overall well-being.

Depending on the program, your organization may operate under ILPTH’s statewide license or your own program license, if applicable. Our team will help determine the best partnership structure for your organization’s needs.

What is a Delivery Partner

A Delivery Partner collaborates with ILPTH to deliver evidence-based programs in local communities.

  • Facilitate workshops using trained staff or volunteers

  • Follow ILPTH guidelines to ensure program quality and fidelity

  • Host workshops at your organization or at partner sites

We work collaboratively with partners to create a structure that aligns with your organization’s capacity and goals.​​

  • Have trained facilitators, complete training through IL Pathways if available, or access certification resources.

  • Maintain program quality and fidelity standards

  • Provide a safe, accessible space for participants

  2. Choose a Program: We’ll work with you to identify the best programs based on your community’s needs and your organization’s capacity.

  3. Training & Implementation: We guide you through the setup process. Some organizations already have trained facilitators. If training is needed, we’ll let you know whether ILPTH offers training for your selected program or connect you with certification resources. You may also view upcoming trainings on our training schedule.

  4. Deliver the Program: Host workshops at your organization or partner with local sites. ILPTH provides ongoing support throughout implementation​​
