Illinois Pathways to Health connects healthcare providers and health plans to evidence-based programs delivered statewide.

 

Most workshops are open to the public and available for referral. Closed workshops are available through contracted health plan partnerships.

Programs Offered

All workshops are evidence-based, delivered in small groups (8–20 participants), and available in-person or virtually.

​Whether patients self-enroll, are referred by providers, or participate through health plan partnerships, Illinois Pathways to Health ensures consistent delivery and measurable outcomes.

 

  • Open Workshops (Referral Pathway) – Participants may self-enroll or be referred by providers.

  • Closed Workshops (Health Plan Pathway) – Offered exclusively to plan members through contracted arrangements.

Available Programs:

  • Self-Management Programs: Build skills to manage chronic conditions, improve independence, and enhance communication with healthcare providers.

  • Falls Prevention Programs: Reduce fall risk, improve balance and strength, and increase confidence for older adults.

  • Caregiver Support Programs: Provide guidance, resources, and strategies to reduce stress and manage caregiving effectively.​

Refer Patients

Open workshops available for patient referrals.

Clinical care does not end after the office visit. Our evidence-based programs help patients build the skills and confidence to manage their health between appointments.

Referral Process:

  1. Submit a secure referral online.

  2. Our team contacts the participant directly.

  3. Participants are connected to the next available workshop.

There is no additional administrative burden for your practice.
