Illinois Pathways to Health, developed and managed by AgeOptions, is a statewide Community Care Hub including social care and evidence-based health promotion programs designed with health providers and insurers in mind. AgeOptions offers a cost-effective contracting solution to any organization that wants to provide social care including HRSN screening, referrals, and health promotion programs on a local, regional, or statewide levels.

We have a track record of successful program implementation, facilitation, data collection, and HIPAA‑compliant data management for health promotion programs, with over 15 years of service to the Suburban Cook County area and beyond.

By contracting with AgeOptions, healthcare providers and insurers can access a statewide network of service providers. Our network consists of Area Agencies on Aging, Care Coordination Units, senior centers, public health departments, and more. Unlike national organizations, we have local expertise to connect clients/members to accessible resources in their community.

HRSN & Referrals

Our trained Community Health Workers utilize the CMS Accountable Communities Health-Related Social Needs Screening (HRSN) screening to assess needs and resolve them. We utilize motivational interviewing techniques, cultural humility, and knowledge of local resources to resolve identified needs.

Workshops can be co-branded with your organization’s logo on promotional materials, participant workbooks, and textbooks. We also offer opportunities for branded consumer goods (e.g., pedometers, hats, bags) as incentives for program completion, reinforcing your brand’s association with health promotion.​

Technology empowerment

Increase your network’s functionality with technology. Online program costs include time to get participants familiar with Zoom through one-on-one technical assistance. This increased knowledge and skill for participants may lead to increased ability to participate in telemedicine visits and utilize Zoom or other online programs to do other health promotion activities and/or engage with others socially.

As an add-on to any workshop, AgeOptions can also complete a social driver of health assessment screen on each registered participant prior to the start of the workshop and make direct referrals to address identified needs.

All the programs listed below are tailored for groups of 8-20 older adults and can be conducted both in-person or virtually—in English with select programs available in Spanish.

We offer these programs in convenient community locations, including senior centers, libraries, faith communities, and clinical settings like hospitals and doctor’s offices.
