Illinois Pathways to Health connects healthcare providers and health plans to evidence-based programs delivered statewide.
Most workshops are open to the public and available for referral. Closed workshops are available through contracted health plan partnerships.
Programs Offered
All workshops are evidence-based, delivered in small groups (8–20 participants), and available in-person or virtually.
Whether patients self-enroll, are referred by providers, or participate through health plan partnerships, Illinois Pathways to Health ensures consistent delivery and measurable outcomes.
