This is the Spanish version of the Diabetes Self-Management Program, designed for Spanish speakers with type 2 diabetes to enhance self-management skills. Participants create weekly action plans, share experiences, and collaboratively address self-management challenges.

About the Class

This engaging program, specifically designed for Spanish speakers with type 2 diabetes, offers a supportive environment where you can connect with others facing similar challenges.

In small groups of 12-16, you’ll be guided by two trained leaders, including peer leaders who understand your experience firsthand. Together, you'll create actionable weekly plans, share your stories, and collaboratively tackle self-management challenges. You'll learn skills and tools to manage your symptoms and keep your blood sugar under control. 

Duration & Format

  • Six weekly sessions, 2.5 hours each (with a built-in break)

  • Participate in person at community and clinical locations or virtually with technical support available

  • Nutrition, healthy eating and menu planning

  • Physical activity and exercise

  • Medication management, monitoring and foot care

  • Working more effectively with health care providers

  • Problem solving, decision making, goal setting and many more
