Who Should Join

  • Adults with ongoing health conditions and their caregivers.

  • Adults whose health problems have begun to interfere with daily activities.

  • Adults who have had challenges following health recommendations.

  • Primary focus: Participants aged 60 and over, as well as adults with disabilities, but individuals of all ages are welcome to join.

  • Caregivers of those with ongoing health conditions will also find the program beneficial.
